PROJECT PERIOD: Aug 2020 – May 2021
DESCRIPTION: A Senior Citizen’s Care Coordination Center (SCCC) was an initiative to strengthen and sustain a local health care program benefitting the vulnerable elderly sector. SCCC was designed in view of the recently passed law on Universal Health Care (UHC) and in response to perceived dwindling health services due to mobility restrictions imposed because of CoVid-19 pandemic. SCCC was envisioned to be a dedicated center for coordinated healthcare delivery for the target patient sector, modeling the Health Care Provider Network (HCPN) as required by UHC> In its initial phase, SCCC provided limited services to respond to the practical needs of the target population, i.e., medication refill for maintenance medications, follow-up diagnostics and treatments, psychosocial support care, emergency care, and referrals from primary care level.
The project implementation experience was expected to provide a model for community-based healthcare along the principles of HCPN and UHC beyond the pandemic. SCCC was piloted in partnership with the Valenzuela Medical Center and the City Health Office of the City Government of Valenzuela.
